Comparison
Cerebrolysin vs Semax
Both are nootropic & neuro, so the meaningful differences are in kinetics, evidence and regulatory status.
On published evidence alone, Cerebrolysin is the better documented of the two (clinical trials). That is a statement about the state of the literature, not a recommendation.

Cerebrolysin
Cerebrolysin is not a single molecule but a standardised enzymatic hydrolysate of purified porcine brain protein, containing low-molecular-weight peptides and free amino acids. It is proposed to act in a neurotrophic-like manner, supporting neuronal survival, synaptic plasticity and neurogenesis in a way that partly mimics endogenous neurotrophic factors. Clinical evidence in vascular dementia and traumatic brain injury exists and is moderately encouraging, though heterogeneity across trials is substantial.
Semax
Semax is an analog of adrenocorticotropic hormone fragment 4-10, extended with proline-glycine-proline to resist enzymatic degradation. The modification removes hormonal activity at the adrenal cortex while preserving central effects, which include increased expression of brain-derived neurotrophic factor and its receptor TrkB, and modulation of dopaminergic and serotonergic transmission. It is on the Russian essential medicines list for stroke and cognitive disorders.
Cerebrolysin vs Semax: common questions
What is the difference between Cerebrolysin and Semax?
Both are nootropic & neuro. Cerebrolysin: A porcine brain-derived peptide preparation used clinically across Europe and Asia for dementia and stroke. Semax: A heptapeptide ACTH fragment analog used in Russia for stroke and cognitive indications, with no corticotropic activity.
Which has stronger evidence, Cerebrolysin or Semax?
Cerebrolysin has the stronger evidence base (clinical trials, about 40 human studies) compared with Semax (preliminary human).
Are Cerebrolysin and Semax FDA approved?
Neither is FDA approved for any indication.
