Gonadorelin vs hCG

Both are reproductive & HPTA, so the meaningful differences are in kinetics, evidence and regulatory status.

Gonadorelin

Gonadorelin is identical to endogenous gonadotropin-releasing hormone, a decapeptide secreted in pulses by the hypothalamus. Pulsatile delivery stimulates pituitary release of LH and FSH, maintaining testicular steroidogenesis and spermatogenesis. The pharmacology is strictly pulse-dependent: continuous exposure downregulates the receptor and suppresses gonadotropins instead, which is how GnRH agonists are used for chemical castration. Its very short half-life is the reason frequent dosing is required.

hCG

Human chorionic gonadotropin is a glycoprotein hormone, not a simple peptide, structurally similar to luteinising hormone and acting at the same LH/CG receptor. In men it stimulates Leydig cells directly to produce testosterone and maintains testicular volume and intratesticular testosterone during exogenous androgen therapy. Because it acts downstream of the pituitary it works even when the HPTA is fully suppressed, which is the practical difference from gonadorelin.

Gonadorelin vs hCG: common questions

What is the difference between Gonadorelin and hCG?

Both are reproductive & HPTA. Gonadorelin: Synthetic gonadotropin-releasing hormone, increasingly used alongside TRT to maintain testicular function. hCG: A placental glycoprotein hormone that mimics LH directly at the testis, the long-standing standard for preserving fertility on TRT.

Which has stronger evidence, Gonadorelin or hCG?

They sit at the same evidence level: regulator approved. Gonadorelin has about 30 human studies and hCG about 60.

Are Gonadorelin and hCG FDA approved?

Both are FDA approved. Gonadorelin: Evaluation of hypothalamic-pituitary-gonadal function (US marketing discontinued). hCG: Prepubertal cryptorchidism; Hypogonadotropic hypogonadism in males; Ovulation induction.
