OneTwenty

Best for: People who want their blood retested every 90 days (a 101-marker baseline plus three follow-up panels a year) and want custom supplements and prescriptions, at published prices, driven by those results in one membership.

Ranks first because bloodwork is the product, not an add-on: four panels a year are included in the $499 membership (a 101-marker baseline plus a follow-up every 90 days), with testosterone cypionate at a published $99/mo, 49 states served and self-serve cancellation. No other TRT clinic here includes repeat labs at that frequency in the base price.

Pricing

Pricing page (seen 2026-11-05; the page says its prices were checked on 2 November 2026): $499 a year, billed annually, for four lab panels: a 101-marker baseline (OneTwenty's own count) plus three follow-up panels of 20 to 40 markers, one every 90 days ('161 to 221 lab results a year'). Walk-in draws at Quest are included; an at-home draw is $99 per draw. NY and NJ: $749 a year because Quest cannot be used there. Add-ons: custom supplement packs from $45/mo; extra lab panels 'at cost' (no prices listed). Medications are priced per product on each treatment page: metformin and sildenafil $49/mo, tadalafil $79/mo, GHK-Cu cream and testosterone cypionate $99/mo, enclomiphene, glutathione (injection or nasal) and oxytocin $129/mo, sermorelin and NAD+ (injection or nasal) $149/mo, compounded semaglutide $179/mo, PT-141 $199/mo, tirzepatide $279/mo or $749 billed quarterly. Whether a separate provider consultation fee applies is not stated. The 'Founding pricing ends at public launch' line seen on 2026-10-17 no longer appears on the homepage or pricing page, but the What We Test page still says 'Now accepting founding members'.

How we ranked these

Every TRT clinic in the directory whose published pricing bundles the initial lab panel, ranked on the standard rubric. Clinics that bill labs separately are listed below the ranking with the reason. Facts come from each provider’s own pricing, FAQ and terms pages, with regulator and court records where they exist. Scores are produced by a fixed table, so the same facts always give the same score. See the rubric.

TRT clinics with bloodwork included: FAQ

What is the best trt clinics with bloodwork included provider?

OneTwenty ranks first, scoring 9.9/10 on our five-criterion rubric. People who want their blood retested every 90 days (a 101-marker baseline plus three follow-up panels a year) and want custom supplements and prescriptions, at published prices, driven by those results in one membership.

How were these providers ranked?

Every provider is scored on the same five weighted criteria: licensed states (24%), labs included (24%), cost transparency (18%), cancellation friction (17%) and formulary breadth (17%). Who prescribes is shown on each review but not scored. Facts come from each provider’s published pages, checked November 6, 2026. Scores marked * are provisional because one or two criteria could not be verified.

Which of these include lab work in the price?

OneTwenty, Lifeforce, Ways2Well, Fountain TRT, Male Excel include the initial lab work in their published pricing. The others bill labs separately or did not publish it.

Do providers pay to be on this list?

No. Placement is set by the rubric score. Where a provider pays us a referral fee, its link is labelled “Paid link”, and that does not change its score.
