Prediabetes and insulin resistance statistics 2026: prevalence, awareness and progression

This page collects government and peer-reviewed data on how many Americans have prediabetes, diabetes and insulin resistance, how many know it, and how often prediabetes progresses. CDC estimates 115.2 million US adults have prediabetes, yet in the most recent national awareness data only 19.0% of adults with prediabetes had been told they have it.

Key findings

  1. CDC estimates 115.2 million US adults have prediabetes, including 52.1% of people aged 65 and older.
  2. Only 19.0% of US adults with prediabetes in 2017-2020 reported being told by a health professional that they had it.
  3. 40.1 million people in the US had diabetes in 2023, and 27.6% of adults with diabetes are undiagnosed.
  4. About 4 in 10 nondiabetic US adults aged 18 to 44 (40.3%) had insulin resistance in 2015-2018, and nearly half of them were not obese.
  5. Between 5% and 10% of people with prediabetes progress to diabetes each year, but lifestyle intervention cut diabetes incidence by 58% in the Diabetes Prevention Program.

Prediabetes and insulin resistance statistics: FAQ

How many Americans have prediabetes?

CDC's National Diabetes Statistics Report estimates 115.2 million US adults aged 18 and older have prediabetes, and CDC says more than 2 in 5 American adults have it. Among people 65 and older, 52.1% have prediabetes.

What percentage of people with prediabetes know they have it?

In CDC's 2017-2020 estimate, 19.0% of adults with prediabetes said a health professional had told them they had it. A 2025 NHANES analysis found 84% of adults with prediabetes in 2011-2020 were unaware, although unawareness fell from 91.55% in 2011-2012 to 79.88% in 2017-2020.

How common is insulin resistance in the US?

Estimates depend on the HOMA-IR cutoff used. A JCEM study found 40.3% of nondiabetic adults aged 18 to 44 had insulin resistance (HOMA-IR of 2.5 or higher) in 2015-2018. Another NHANES analysis found 38.4% of nondiabetic adults had insulin resistance in 2017-2018, up from 24.8% in 1999-2000.

What percentage of people with prediabetes develop type 2 diabetes?

A Lancet review reports that 5% to 10% of people with prediabetes progress to diabetes each year, and cites an American Diabetes Association expert panel estimate that up to 70% eventually develop diabetes. In the Diabetes Prevention Program placebo group, 62% had developed diabetes by year 15.

Can prediabetes be reversed or delayed?

In the Diabetes Prevention Program trial, intensive lifestyle intervention reduced type 2 diabetes incidence by 58% and metformin by 31% compared with placebo over an average of 2.8 years. After 15 years, the lifestyle group still had 27% lower incidence than placebo.
