TRT statistics: FAQ
How many men in the US are on TRT?
There is no single national count. Epic Research found 0.93% of male patients had an active testosterone prescription in 2025 and 0.95% in the first half of 2026. State prescription monitoring data counted 1,656,641 people filling testosterone prescriptions in 2022 in the states studied, up from 1,216,982 in 2018. FDA reported 2.3 million patients received a testosterone prescription in 2013.
Is testosterone prescribing increasing?
Yes, after a decline. Use among men 30 and older peaked at 3.20% in 2013 and fell to 1.67% by 2016. In Epic Research data the share of male patients on testosterone fell to 0.64% in 2021, then rose to 0.93% in 2025.
Which men are most likely to take testosterone?
Men aged 50 to 64 have the highest rate, with 1.57% holding an active prescription in 2025, followed by men 40 to 49 at 1.35%. Men 18 to 29 have the lowest rate at 0.14%.
Do doctors always test testosterone before prescribing TRT?
Not always. In 2001 to 2011 claims data, only 74.72% of new androgen users had a testosterone level measured in the prior 12 months, and a later study found more than 20% of men aged 40 to 65 had no lab test before starting TRT.
Does TRT raise the risk of heart attack or stroke?
In the TRAVERSE trial of 5,246 men with low testosterone and high cardiovascular risk, major adverse cardiac events occurred in 7.0% of the testosterone group and 7.3% of the placebo group. On February 28, 2025, FDA moved to remove Boxed Warning language on increased cardiovascular risk while adding blood pressure warnings to all testosterone products.
Can you get TRT through telehealth?
Yes. Testosterone is a Schedule III controlled substance, and DEA and HHS have extended telemedicine flexibilities allowing video prescribing without a prior in-person visit through December 31, 2026. A 2022 secret shopper study found 6 of 7 direct-to-consumer platforms offered testosterone to a man with normal levels, so choosing a service that follows clinical guidelines matters.
