The best peptide therapy clinics that prescribe rather than sell
A prescribing clinic can only offer compounds a pharmacy may legally compound, which after the FDA 503A Category 2 decisions excludes BPC-157, ipamorelin and CJC-1295. The clinics here work inside that, which is the point.
Ranked first on the six-criterion rubric, at 7.1/10.

- Trustpilot 4.9 from 989 reviews; BBB A+ with 2 complaints in 3 years
Watch out for
- Lab panel is a separate purchase (minimum $450) and outside lab results are not accepted to start
- Medication prices are not published, and the meaning of the $299 figure is not explained
- All web sales are final and non-refundable
- Requires an in-person physical exam from an outside clinician every 12 months
Cancelling
FAQ: 'we don't offer any set subscription plans and you can cancel whenever you want.' Cancellation & Refund Policy: 'All Marek Health web sales are final and non-refundable'. The booking payment is a non-refundable deposit toward the provider exam. Appointment changes need 24 hours' notice by email, text or phone, or a fee of up to $50 may apply; no-shows may also be charged up to $50.

FAQ: Guided Optimization 'is available in all 50 states across the United States. However, specific treatments and diagnostics may be limited to certain states.' DC is not mentioned in the FAQ (the sign-up form's state list includes DC).

TRT tiers are Flex at $149/mo (3-month commitment, $447 upfront), Commit & Save at $129/mo (6 months, $774 upfront) and Full Year at $99/mo (12 months, $1,188 upfront). The page says the fee covers medications, clinician consults and follow-up labs. A lab panel and clinician visit are required before starting: the Complete Hormone Panel is $149 and the Advanced Panel is $399, and an at-home phlebotomist costs $99 where available.

What we like
- TRT tiers, lab panel prices and restricted states are all published on the product page
- The monthly TRT fee covers medication, clinician consults and quarterly follow-up labs
- BBB A+ and accredited with 0 complaints in 3 years; Trustpilot 4.5 from 1,433 reviews
- Patients can choose asynchronous or synchronous consultations
Watch out for
- The advertised $99/mo TRT price requires paying 12 months ($1,188) upfront; the shortest plan is 3 months at $149/mo
- Initial labs are extra ($149-$399), and all sales are non-refundable
- TRT is unavailable in 16 states, including PA, NC, GA and MN
- Does not bill insurance; peptides and labs are not covered
Cancelling
TRT plans are sold with 3-, 6- or 12-month commitments paid upfront. The Terms also require at least three consecutive months of subscription and an email cancellation at least 30 days before the next processing date, but that section is bracketed and may be draft text. The help center says subscriptions can be cancelled any time in the Patient Portal (Home > Subscriptions) before the next renewal date. All sales are final: medication, subscription and lab payments are not refunded.
Where it is available
TRT cannot be prescribed or shipped to AL, AR, CT, DE, GA, HI, LA, MN, MO, MS, NC, ND, OK, PA, RI or SC (16 states), which leaves 34 states. The help center says testing and treatment are offered 'in all fifty states' across its therapies. DC is not mentioned for TRT.

The annual membership includes a 101-marker baseline panel plus three targeted 20-40 marker follow-up panels, device integration, AI health score, a health coach and custom supplements. Prescription medications are billed monthly or quarterly 'from $49/mo, on top of the annual membership'. The site says 'Founding pricing ends at public launch', so $499 may be a founding-member rate.
Prices as listed on October 17, 2026. They change often; confirm at checkout.
What we like
- Four lab panels a year (101-marker baseline plus three follow-ups) are included in the membership price
- Price is published up front: $499/yr ($749/yr in NY/NJ), with medications 'from $49/mo'
- Lists 49 states (all but Rhode Island)
Watch out for
- The annual membership is non-refundable after a 7-day window, and only if no service has started (3% fee applies)
- Not a medical practice: prescriptions come from unnamed third-party telehealth providers, and the site does not say whether they are physicians or NPs or whether visits are by video
- Early-stage company: it opened a closed beta of 25 founding members in December 2025, and the site says founding pricing ends at public launch
- No direct insurance billing (HSA/FSA reimbursement only)
Cancelling
Payment and Refund Policy (updated 2026-07-28): cancel via the option in account settings or by emailing support@onetwenty.com. The annual membership renews automatically. A refund is available only within 7 days of purchase and only if no service has started, minus a 3% fee. After that the annual fee is non-refundable, except for a discretionary prorated refund if no lab is reachable. Medication add-ons are non-refundable once prescribed.
Where it is available
Site FAQ: 'available in 49 states except for Rhode Island'; NY and NJ carry a higher fee ($749/yr) because of state lab-draw rules. DC not mentioned.

Defy's FAQ estimates male TRT averages under $175/month in the first year and about $125/month after that. The estimate covers testosterone, a weekly aromatase inhibitor, labs and consultations, and Defy says it is not a price guarantee. There is no monthly fee: consultations, labs and medications are each paid for when bought. No consultation fee is published on Defy's site.
Prices as listed on October 17, 2026. They change often; confirm at checkout.

- Care team of physicians, physician assistants and nurse practitioners, plus an in-person clinic in Tampa
- Trustpilot score of 4.8 from 4,221 reviews
Watch out for

- Does not accept insurance of any kind (it provides a superbill on request)
- Its Peptide Therapy service page features BPC-157, TB-500, Epitalon, KPV and MOTS-c, which are on FDA's 503A Category 2 safety-risk list
- BBB rating of C- (not accredited), with 4 complaints in the last 3 years
Cancelling
No subscription or contract: 'Defy Medical doesn't lock patients into subscriptions or contracts', and charges are due when a service is purchased, so there is no recurring plan to cancel. Appointments cancelled with less than 24 hours' notice, and no-shows, may be charged the full consultation fee. Sales are generally final. Lab orders are refundable only before specimens are submitted, and dispensed medications are not refundable.
Where it is available
Says telemedicine is offered in 'most states throughout the US' without listing them. Its lab-ordering site (testdefy.com) says on-demand lab testing is not offered in NY, NJ, PA, RI or MA. The ToS says controlled-substance prescriptions are not sent across state lines.

How we ranked these
Same rubric. Clinics advertising Category 2 compounds are excluded. Facts come from each provider’s own pricing, FAQ and terms pages, with regulator and court records where they exist. Scores are produced by a fixed table, so the same facts always give the same score. See the rubric.
Best peptide therapy clinics: FAQ
What is the best peptide therapy clinics provider?
Marek Health ranks first, scoring 7.1/10 on our six-criterion rubric. Data-driven clients who want extensive lab panels and coach-led, provider-supervised hormone optimization, and who will pay out of pocket without a clear upfront total.
How were these providers ranked?
Every provider is scored on the same six weighted criteria: licensed states (20%), labs included (20%), prescriber credentials (15%), cost transparency (15%), cancellation friction (15%) and formulary breadth (15%). Facts come from each provider’s published pages, checked October 20, 2026. Scores marked * are provisional because one or two criteria could not be verified.
Which of these include lab work in the price?
OneTwenty, Ways2Well include the initial lab work in their published pricing. The others bill labs separately or did not publish it.
Do providers pay to be on this list?
No. Placement is set by the rubric score. Where a provider pays us a referral fee, its link is labelled “Paid link”, and that does not change its score.
